STOP GAP DENTAL PRIVACY POLICY

Stop Gap Dental offers various services to connect Dental Practices with Individual Contractors. Stop Gap Dentals Privacy Policy (“Privacy Policy”) is part of our Terms and Conditions, which you can view here, and is designed to explain what information we collect and how we use it to improve your experience. This Privacy Policy applies to any User of SGD’s Services and/or Site.

For the purposes of this Privacy Policy, “SGD, “we,” “us,” and “our” refer to Stop Gap Dental, LLC. “User,” Users,” “you,” and “your” refer to any visitor to SGDs Site or registered user of SGD’s Services. “Services” refers to all services and systems offered by SGD, through any platform, including our web page, mobile application, mobile websites, email, and/or telephone system. “Site” refers to our web page and mobile websites.

By using the Services or visiting the Site, you consent to our collection, storage, use and disclosure of your information as described in this Privacy Policy.

Information We Collect

Information you Disclose

In general, you can visit the Site without revealing any information about yourself. However, all information you provide us for the purposes of using the Services or otherwise will be stored by us. You represent and warrant to us that you have the right and authority to provide any and all information you provide about yourself or others.

When you register for an SGD account, we collect information from you that may include your name, company name, email address, telephone number, street address, and other information that you choose to submit as part of your profile or job posting. By creating an account and providing your information to us, you explicitly agree to our collection and use of such information as described in this Privacy Policy and understand that while some information is optional, certain information is required for SGD Services to fully function. Therefore, your failure to provide certain information will restrict your ability to create an account.

When you post a job, create a profile, or otherwise communicate through the Services, you may provide us with additional information including but not limited to the type of contractor you seek, the type of position you seek, hourly rate information, pictures, and references. By disclosing this information through your use of the Services, you explicitly agree to our collection and use of such information as described in this Privacy Policy.

When you contact us through any platform, you may be asked to provide information including but not limited to your name, telephone number, email address, street address, and company name for account security and verification. By disclosing this information to us, you explicitly agree to our collection and use of such information as described in this Privacy Policy.

Information Provided by Others

Other Users may provide information about you including but not limited to a star-based review. Further, third parties may provide us with information about you to verify your information and representations. We may capture and store all information we receive from Users and third parties about other Users.

Information Automatically Collected

We may automatically collect information about how you use the Services. This information may include statistical information and usage information such as:
    • Device-specific information, such as hardware model, operating system version, unique Device identifiers and mobile network information, including your phone number, Internet Protocol (“IP”) address, and device event information such as crashes. For the purposes of this Privacy Policy, “Device” means any computer used to access the Services, including but not limited to a desktop, laptop, mobile phone or tablet.
    • Locale preferences.
    • Application activity.
    • Date and time stamps associated with transactions.
    • System configuration information.
    • Metadata concerning your mobile or smartphone applications.
    • Information related to the way you interact with the Site and Services, such as referring and exit pages, URLs, number of clicks, and content viewed, amount other similar information.

Cookies and Analytics

We use cookies and tracking pixels to collect and aggregate information about the Services. A cookie is a tiny data file that allows us to recognize when you return to the Site using the same Device and browser. By using our Site, you will be deemed to have consented to the placement of cookies on your Device. Further, we use Google Analytics to help us understand our Users’ experience. The features we use allow us to generate reports on Users and created advertising materials based on such metrics. Visitors can opt-out of Google Analytics for Display Advertising and customize Google Display Network ads using the Ads Setting form here.

Do Not Track Notice

We currently do not use technology that recognizes a “do-not-track” signal from your browser. Should such technology become available to us, we will not honor such browser requests, but we will not use your information in a manner that violates this Privacy Policy. You retain the ability to opt-out of certain online analytics and advertising activities as discussed above.

Protecting Children’s Privacy

This Site and the Services are not designed or intended to be attractive to use by children under the age of 18 years old (“Minors”). We do not knowingly collect information from minors. If you are a Minor, please do not submit any information to us and/or use the Site or Services. If we become aware that we have collected personal information from Minors, we will take reasonable steps to promptly delete the account and all information directly linked to said account.

Use of Information

The information we collect is generally used to improve the Services and the Site and to help us communicate with you. For example, we may use your information to register and enable your account, connect you with other Users, perform analytics, notify you of new features or other information with which believe will be of interest to you, personalize promotional messages, generate reports about data usage and patterns, and verify information you provide to us, among other things.

We may aggregate and/or de-identify the information we collect from you. This Privacy Policy does not limit our use and disclosure of aggregated and/or de-identified information. We may use it without limitation for any purpose.

Disclosure of Information

In addition to using your information as described above, we may also reveal information about you to third parties if:
    • You request or authorize it.
    • Providing the information will help us complete a transaction for you, provide the Services to you, and/or improve your ability to use the Site.
    • It is necessary to comply with the law, regulations, governmental and quasi-governmental requests, court orders or subpoenas, to enforce the Terms and Conditions or other agreements, or to protect our rights or the rights of our Users.
    • The disclosure is part of a purchase, transfer or sale of services or assets.
    • Doing so will allow our agents to perform functions on our behalf.
    • The disclosure allows our vendor, service providers or agents to market to you.

If you terminate your registration for any reason, we reserve the right to notify other Users with whom we reasonably believe you have communicated of your termination.

Your Choices

Users choose what additional information they provide to us, including but not limited to pictures and details in a job description or profile. In addition, Users can opt-out of the use of cookies and google analytics as described above. Users can opt out of receiving promotional offers by following the “unsubscribe” link at the bottom of any promotional email or contacting us directly.

Accessing Your Personal Information

By using our Site and the Services, you agree to provide us with accurate and current information. You can access your personal information at any time through your account and can modify or delete your information by choosing the edit function on your profile within the Dashboard.

If you would like to remove content you have posted to the Site, such as in the Resource Center, contact us via email. In some cases, we may not be able to accommodate your request, but if we are unable to do so we will explain why.

If you no longer wish to use the Services, you may close your account. We will remove your name and other contact and identifiable information from publicly viewable forums, although any content you may have voluntarily posted, such as comments in an online forum, will remain visible. We have no obligation to retain your information and may delete any and all information without liability. We may, however, retain your information for any and all legitimate business purpose, including providing the Services to other Users, for as long as we believe it is necessary. You can inquire whether we are retaining any of your personal information and request a copy of all personal information we have retained by contacting us via email. We may require you to provide proof of your identity before we provide you with a response.

Security

SGP implements reasonable administrative, physical and technical safeguards to protect your personal information against unauthorized access, misuse or disclosure. Although we take appropriate measures to protect your information, we cannot guarantee that it will never be disclosed in a manner that is inconsistent with this Privacy Policy. Therefore, we recommend that you safeguard your user name and password and notify us immediately of any unauthorized use of your account or any other type of breach of security. We reserve the right to refuse service, suspend and/or terminate accounts, or remove or edit content in our sole discretion. Any use of the Services and/or Site which is deemed to be bullying or harassment will not be tolerated and will result in immediate suspension and/or termination of the account.

California Privacy Rights

California law permits California residents who are Users to obtain a list of third parties to whom we have disclosed their personal information for the purposes of direct marking in the prior calendar year. Links to Other Websites

When you use the Site or Services, you may choose to follow links to other websites that are beyond our control. We are not responsible for the privacy practices employed by those web sites or services, nor are we responsible for the information they contain. This Privacy Policy applies solely to information collected by us through the Site and Services.

Changes

We reserve the right to change this Privacy Policy and our Terms and Conditions at any time. We will notify you of any material changes as required by law. Material changes will go into effect 30 days after notification, and non-material changes will take effect immediately upon posting of the updated policy. Your use of the Site and/or the Services after such effective date is deemed to be an acceptance by you of such changes.
